Sleep Quality, Cognition, and Disease Severity in Parkinson's Disease
Parkinson Disease

The goal of this observational study is to investigate the relationship between sleep quality, cognitive functions, and disease severity in individuals with Parkinson's disease. The main question it aims to answer is: Is sleep quality associated with cognitive function and disease severity in individuals with Parkinson's disease? Participants will complete assessments of sleep quality, cognitive function, disease severity, and disease stage during a single evaluation session. Cognitive function will be evaluated using the Stroop Test, Clock Drawing Test, and Montreal Cognitive Assessment. Disease severity will be assessed using the Unified Parkinson's Disease Rating Scale, and disease stage will be determined using the Modified Hoehn and Yahr Scale.

Leave Nothing Behind Study Which Compares DCB With Bail Out BRS Versus BRS Strategy Alone
Drug Coated Balloon Bioresorbable Scaffold Percutaneous Coronary Intervention (PCI) Acute Coronary Syndrome (ACS)

The goal of this study is to investigate the equivalence in early and long-term efficacy between the two "Leave nothing behind strategies" (Drug-Coated Baloon \[DCB\] strategy with bail-out BioResorbable Scaffold \[BRS\] versus BRS strategy) of de-novo native coronary artery lesions in a relatively young Percutaneous Coronary Intervention (PCI) population, to be more specific, Patients with Chronic Coronary Syndromes (CCS) and Acute Coronary Syndrome (ACS) (Non-ST-segment Elevation Myocardial Infarction \[NSTEMI\] and Unstable angina) between 18-68 years of age scheduled for PCI. The main questions aim to answer are: DCB strategy with bail-out BRS implantation has equivalent clinical outcomes at 12 months compared to BRS strategy? DCB strategy with bail-out BRS implantation has noninferior angiographic in-segment net gain at 13 months compared to BRS strategy? DCB strategy with bail-out BRS implantation has equivalent clinical outcomes at 60 months compared to BRS strategy? HBM-Based Solution-Focused Birth Program on Fear of Childbirth in Primigravidas
Fear Stress (Psychology) Pregnancy

Pregnancy is a period characterized by significant physiological and psychosocial changes, during which women adapt to new roles. While some women experience this period with excitement and hope, others may experience anxiety, uncertainty, and fear. Fear of childbirth (tokophobia) is a common condition that can negatively affect women's quality of life, birth preferences, and childbirth experiences. High levels of fear of childbirth have been associated with increased cesarean section rates, negative birth perceptions, and postpartum psychological problems. This randomized controlled trial aims to evaluate the effect of a solution-focused childbirth preparation education program based on the Health Belief Model on fear of childbirth and belief in normal birth among primigravida women. Secondary outcomes include pregnancy-related stress and mode of delivery. Participants will be randomly assigned to either an intervention group receiving structured education or a control group receiving routine prenatal care. Data will be collected before and after the intervention using validated measurement tools, and delivery outcomes will be recorded after birth. The findings are expected to contribute to improving maternal psychological well-being and promoting positive childbirth experiences.

Oncology Acute Care Follow-up Intervention Study
Cancer

This pragmatic randomized controlled study evaluates an education intervention designed to increase patient engagement with Oncology Acute Care (OAC) services among patients being treated for cancer at Parkland Health. Eligible patients will be randomized to either a control (usual care) arm or an intervention arm. If a patient in the control group has experiences a subsequent cancer treatment related ED visit without documented OAC engagement, they will be moved to the intervention arm. Patients in the intervention arm will receive an automated MyChart message and text message within 24 hours of hospital discharge reinforcing oncology acute care (OAC) resources, including contact information for the triage line and guidance for when to seek urgent versus emergency care. Messages will be resent at 48 and 72 hours if unviewed. Once the message is viewed, the patient will enter an outcome tracking period to monitor time to subsequent ED visit or OAC contact. Follow-up will occur in 3-month intervals until an outcome is documented or the study ends. After randomization, patients will be monitored on an observation list for outcomes including OAC contact and cancer treatment-related Emergency Department (ED) visits. Patients who contact OAC clinic will be removed from observation list and complete the study. We will repeat the observation period and document outcomes if the patient has a subsequent cancer treatment-related ED visits without documented OAC engagement.

AMPLIFI: Adaptive Modulation of Plasticity Through Lactate and Fitness Interventions
Stroke, Chronic Aging Cognitive Decline Motor Dysfunction +1

The AMPLIFI study (Adaptive Modulation of Plasticity through Lactate and Fitness Interventions) investigates how short-term aerobic exercise influences brain plasticity and learning in older adults and stroke survivors. The study compares three groups: one performing aerobic cycling at an intensity that elevates lactate levels, one performing low-intensity exercise, and one receiving health education without exercise. All participants will complete motor learning tasks and undergo brain-stimulation testing using transcranial magnetic stimulation (TMS) to assess how well the brain responds to training. The goal is to understand whether different types of exercise can improve brain function, movement, and memory, and how the body's response to exercise (like lactate levels) might support brain health. This research may help identify low-cost, non-invasive interventions-such as targeted exercise-that improve motor and cognitive outcomes in aging and stroke recovery.

A Clinical Trial of MK-4646 With Bictegravir/Emtricitabine/Tenofovir Alafenamide and Dolutegravir in Healthy Adult Participants (MK-4646)

Researchers are looking for new treatments for people living with HIV-1(Human Immunodeficiency Virus Type 1). HIV-1 is the most common type of HIV, which is a virus that attacks cells of the immune system. HIV-1 treatments, called ART (antiretroviral therapy), involve taking medicines to lower the amount of HIV-1 virus in the body. Standard ART may include Bictegravir/Emtricitabine/Tenofovir Alafenamide (BIC/FTC/TAF) and Dolutegravir (DTG). MK-4646 is a trial medicine designed to treat HIV-1. Before giving a trial medicine to people with a health condition, researchers first do trials in healthy people. The goals of this study are to learn: * If taking MK 4646 together with BIC/FTC/TAF or DTG changes the amount of these ARTs in the blood over time. * About the safety of MK-4646 and if people tolerate it. Effects of Conditioning Activities in Female Athletes
Placebo Effect Physical Performance Countermovement Jump

This study will investigate how different warm-up strategies affect physical performance in female athletes who practice invasion sports (e.g., soccer, handball, and basketball). Participants will complete three different conditions: a standard warm-up only, a warm-up followed by a performance-enhancing activity, and a warm-up followed by a low-intensity activity designed to simulate the same expectations without real physiological effects. After each condition, athletes will perform tests of vertical jump performance and change-of-direction speed. In addition, participants will report their perceived effort, expectations, muscle soreness, and recovery status. The study will include eighteen female athletes and will be conducted under controlled conditions, including standardized hydration, recovery, and environmental factors. The design will allow comparison of the physical and psychological effects of the different warm-up strategies on performance.

Venous Compression in Fontan

The aim of this study is to investigate whether venous compression garments increase exercise stroke volume in patients with Fontan circulation. To address this aim, we will test the following hypotheses: 1. Acute and chronic external venous compression will increase exercise stroke volume in patients with Fontan circulation. 2. Patients with significant venous varicosities will have a greater response to venous compression. Participants will: * Undergo submaximal exercise testing in MRI to measure venous return and exercise stroke volume with and without the wearing of compression garments * Undergo submaximal exercise testing on a seated upright exercise ergometer with concurrent measurement of stroke volume with and without the wearing of compression garments * A subset of participants will repeat both testing visits after wearing compression garments for 2-weeks during waking hours. Participants will: * Undergo submaximal exercise testing in MRI to measure venous return and exercise stroke volume with and without the wearing of compression garments * Undergo submaximal exercise testing on a seated upright exercise ergometer with concurrent measurement of stroke volume with and without the wearing of compression garments * A subset of participants will repeat both testing visits after wearing compression garments for 2-weeks during waking hours.

Effect of Sildenafil on Left Ventricular Function in Pediatric With Primary Dilated Cardiomyopathy

Dilated cardiomyopathy (DCM) is a clinical diagnosis characterized by left ventricular or biventricular dilation and impaired contraction that is not explained by abnormal loading conditions.(1) Both inherited predisposition and environmental factors play an important part in the natural history of disease.(1) Cardiomyopathies are group of heart diseases that influence cardiac muscles directly and are not related to hypertension, congenital, valvular and pericardial diseases. The most common type of cardiomyopathy is dilated cardiomyopathy (DCM).(2) Definitions of DCM provided by two major professional organizations (American Heart Association (AHA) and European Society of Cardiology (ESC)) Both organizations agreed that DCM could be clinically defined by the presence of left ventricular dilation and contractile dysfunction in the absence of abnormal loading conditions and severe coronary artery disease.(3) Dilated cardiomyopathy usually manifests as chronic systolic heart failure, which is detected by echocardiography as impaired left ventricle fraction shortening less than 28%, with left ventricular end diastolic dimension Z-score\>2 ,leading to arrhythmias and sudden death.(4,5) idiopathic dilated cardiomyopathy (DCM) is characterized by dilatation and impaired contraction of the left ventricle or both ventricles, in the absence of underlying causes such as CAD, valve disease, congenital heart disease, or pericardial disease. Pharmacogenetics and Model-Informed Optimisation of Hydroxyurea
Sickle Cell Disease

The wide interindividual variability in clinical response to hydroxyurea therapy in the management of sickle cell disease has limited its use. These variabilities have been linked to differences in pharmacodynamics, pharmacokinetics, and pharmacogenetics. This study, therefore, aims to enhance understanding of these factors as they relate to hydroxyurea therapy, with the overall goal of developing a precision medicine algorithm. The study will be a prospective cohort pharmacokinetic study of 100 Nigerian patients with sickle cell disease, including current hydroxyurea users and naive patients. Pharmacodynamic markers will be collected to evaluate response. PopPK and PK-PD models will be developed in Monolix, exposure-response relationships will be analysed in R, and pregnancy, lactation, and paediatrics PBPK models will be developed in Simcyp or PK-SIM to inform dose optimisation. This study aims to build a pharmacometric model by integrating differences in pharmacokinetics, pharmacodynamics, and pharmacogenetics of hydroxyurea, which could aid the optimisation of hydroxyurea for sickle cell patients in Nigeria. The objectives of the study are i. To determine the prevalence of genetic polymorphisms in metabolic enzymes and transporters relevant to the disposition of hydroxyurea in the Nigerian sickle cell disease population, ii. To develop and validate an analytical method for the quantification of hydroxyurea using high-performance liquid chromatography. iii. To evaluate the influence of genetic and other covariates on hydroxyurea disposition in the Nigerian sickle cell disease population using population pharmacokinetic modelling, iv. To investigate the relationship between hydroxyurea exposure and clinical outcomes (foetal haemoglobin, mean corpuscular volume, reduction in vaso-occlusive crises (VOC), and improved blood count) using pharmacokinetic-pharmacodynamic modelling, v. To develop physiologically-based pharmacokinetic (PBPK) models that could predict hydroxyurea concentrations in special populations of sickle cell disease patients in Nigeria i.e. pregnant women, lactating mothers, breastfed infants, and paediatrics. vi. To develop a dosing guideline for hydroxyurea therapy in Nigerian sickle cell patients. Overall, this study will provide scientific knowledge that can enhance clinical decision-making in sickle cell management within the Nigerian population, and the models could serve as a template to optimize hydroxyurea use in this population.

GAPP Induction and Concurrent Chemoradiotherapy for High-risk Locoregionally Advanced NPC.
Nasopharyngeal Carcinoma

The investigators have designed a multicenter, open-label, randomized controlled phase III clinical study of GAPP induction therapy followed by concurrent chemoradiotherapy and toripalimab maintenance therapy for high-risk locoregionally advanced nasopharyngeal carcinoma (stage III, AJCC 9th edition). The aim is to obtain high-level, high-quality evidence-based data to clarify the efficacy and safety of combining chemoradiotherapy with PD-1 antibody and anlotinib, thereby providing a new treatment strategy to improve the prognosis of patients with high-risk locoregionally advanced nasopharyngeal carcinoma. In this study, GPP induction chemotherapy followed by concurrent chemoradiotherapy and toripalimab maintenance therapy is selected as the control group. This regimen is currently the standard treatment recommended by guidelines for high-risk locoregionally advanced nasopharyngeal carcinoma, with well-established efficacy and broad clinical application. It provides a reliable benchmark for comparing the efficacy and safety of the experimental group, meets ethical requirements, and has mature clinical operational procedures.

Early Signals of the Transition From Immune Quiescence to Activation in the Liver Allograft Microenvironment and in the Circulation

This is a prospective multi-center, longitudinal study to determine efficacy of 50 percent Immunosuppression (IS) reduction. One hundred fully eligible participants will reduce IS by 50 percent in two steps. Liver tests will be checked every 0.5 months through month 4, once a month through month 12, and every other month through month 18. Liver transplant (LTx) center visits will take place at screening, months 6, 12 and 18 after initiating IS dose reduction. A protocol driven liver biopsy to adjudicate the endpoint will be performed at 18 months. The duration of the study from time of starting IS dose reduction to the primary endpoint assessment is 18 months. The primary objective is to assess the efficacy of 50 percent IS dose reduction in children with Liver transplants (LTxs)
